Visit the HACH to pick up an application, located at 150 Roebuck Court, Hazlehurst, MS, between the hours of 8:00 am until 5:00 pm.Once the application has been completed, it must be hand delivered to the address listed above. No documents are needed at this time.The HACH does not have a preference.
