Visit the HDGA to pick up an application, located at 101 Stansbury Court, Havre de Grace, MD 21078, Monday-Thursday between the hours of 9:00 am and 3:00 pm.2. Request an application be emailed to you by calling 410-939-2097 during normal office hours.Once the application has been completed it can be mailed or hand delivered to the address listed above.
