Visit the HAS to pick up a pre-application, located at 41 Cardinal Dr, Shelbyville, KY 40065 during normal office hours.2. Request to have a pre-application mailed to you by calling (502) 633-4531 during normal office hours.Once the pre-application has been completed, it must be hand delivered to the address listed above during normal office hours.Be sure to include these documents with the pre-application: Social Security Cards and birth certificates for all household members, proof of all income, and photo IDs for all adult household members.The HAS does have preference for: Applicants that live and/or work in Shelbyville, elderly, disabled, or employed.For more information, call the HAS office at (502) 633-4531 during normal office hours.
